It's thought to be a programmer's joke, the ladder isn't climbable. If you get up there with the position editor or the fly cheat, you'll find a large med-pack.

Yes, but near the beginning, the runner uses a tremendous shortcut that skips about 80% of the level. The shortcut is NOT based on a bug, by the way. Since Louis Chou planned to get all secrets, he couldn't take the shortcut (which skips two secrets).
